2. Provide disability certification form.
Certification Form for TAP Programs
To process your application, we must have a valid disability certification on file. If you have not submitted a certification within the last three years, you will need to provide a new one. A list of approved professionals who can sign
off on your disability is included directly on the form.
If you think your records are already on file with us, please call to verify that they are still current and to see if anything else is needed.

What happens next?
Once we receive your completed and signed application and verify your disability certification, the TAP-I team will review your application. If the equipment is approved, we will order it and ship it directly to your email address (for software)
or home address (for hardware).
Most software carries a multi-year license, and many devices come with warranties. If you experience any difficulties with your equipment or software, please contact TAP before discarding the item.
In most cases, applicants who need new or updated equipment may reapply after three years. If a change in disability occurs, please contact the program to discuss options.
While this equipment is provided completely free of charge, we do have one request: Program participants are expected to complete and return the TAP-I feedback surveys that we send out via email.
